Patient Information

Your First Visit

Your initial appointment will consist of a thorough examination explaining your dental concerns, your diagnosis and treatment options. Occasionally, treatment can be done the same day. A complex medical history or treatment plan may require an evaluation by a specialist and a second appointment to provide you with the necessary treatment.

Please assist us by providing the following information at the time of your consultation:

  • Any X-rays if applicable.
  • A list of medications you are presently taking.
  • If you have dental insurance, please bring your insurance card or information.
  • IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

*Please alert the office if you have a medical condition that may be of concern proir to any dental treatment (i.e. diabetes, high blood pressure, artifical heart valves, and artifical joints, rheumatic fever, Osteoporosis etc.) Requiring premedication or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)


If your previous dentist has taken recent x-rays (within 6-months), you may request that they forward them to our office at [email protected] If there is not enough time, please pick them up and bring them to our office. If additional films are necessary for diagnosis, they can be taken at our facility.

Financial Policy

For your convenience we accept Visa, MasterCard, Discover, American Express and Care Credit. We deliver excellent care at the most reasonable cost to our patients. Therefore payment is due at the time service is rendered. If you have questions regarding your account, please contact us at 520-797-1240.

Please remember you are entirely responsible for all fees charged by this office regardless of your insurance coverage. It is the responsibility of the patient to know their individual insurance coverage. Regarding payments, most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.

To expedite your appointment, please fill out the Patient Information and HIPAA forms below. You may complete these forms online—they will be sent directly to our office—or you may download, print, complete, and bring them with you to your scheduled appointment.
